Introduction. Anosmia and ageusia are one of the most common characteristic symptoms severe acute respiratory syndrome coronavirus 2 (SARS-Cov-2) infection, with a frequency almost 50% in patients Western countries. Hypotheses proposing that virus potentially affects central nervous system (CNS) on rise. One hypothesis suggests enters via nasal mucosa then olfactory bulb cribriform plate, further dissemination to CNS. Case report. A 34-year-old female patient experienced loss sense smell taste about two months before testing positive for SARS-Cov-2. Coronavirus disease 2019 (COVID-19) presented minor pneumonia worsening anosmia ageusia. After treatment, recovered well, but appeared again, varying intensity, since February 2021, they have become persistent. The case was evaluated by an otorhino-laryngologist, pulmonologist, finally, neurologist. In meantime, tested negative SARS-Cov-2 received doses Sputnik V vaccine. Brain magnetic resonance imaging (MRI) performed, it clearly showed bilateral atrophy. has had up this day, future MRI follow-up is planned. Conclusion. Loss may be predictor CNS dis-semination possible neurological complications (which still subject consideration). could gateway COVID-19 intrusion into CNS, its atrophy indicator process. Further investigation topic required, including wide application MRI, order come definite conclusions.
